Monash University Supplementary Information Forms
As part of the VTAC application process a number of Monash University courses require applicants to complete a Supplementary Information Form. These forms vary from course to course, faculty to faculty. These forms are compulsory, if you do not complete the form or fail to complete it correctly, you may not be assessed for admission into the course of your choice.
