An original version of your Student Response Form will be sent to you with your Notice of Referral and Hearing. This original version will contain the important dates of submission you need to reply by.  The following generic documents do not contain the dates.  Please contact your faculty office for these or refer to your Notice of Referral and Hearing.

Version A - for the Faculties of Arts; Education; Medicine, Nursing and Health Science; Science.

Version B - for the Faculties of Art and Design; Business and Economics, Engineering; information Technology; Law; ; Pharmacy and Pharmaceutical Sciences.

Students requiring advice or assistance regarding the Academic Progress Committee process (including Hearings and Appeals to the Dean) should contact their faculty administration office on their home campus.

Students are also able to obtain assistance from Student Rights Officers on each campus for advice on how to respond to a Notice of Referral and Hearing, Hearing preparation and/or Appeals against Exclusion.
